At 4C Impact, we understand that health and social care systems are at a crossroads—facing immense challenges, yet offering remarkable opportunities for transformation.
My mission is to help leaders, organizations, and communities reimagine what’s possible by uniting people and systems to ignite meaningful, lasting change.
Over my 30 years of experience, I’ve learned that the most profound transformation happens with people, not to them. The answers we seek are already within us—in our communities, our relationships, and the collective wisdom we unlock when we worktogether. Real change demands more than plans and timelines; it calls for new ways of thinking, collaborating, and creating.
At 4C Impact, we specialize in system orchestration—a process that bridges divides and aligns people, resources, and goals to achieve collective impact. In a fragmented health and social care landscape, system orchestration creates the pathways to overcome barriers, harmonize efforts, and co-create sustainable solutions that serve individuals and communities alike.Collaboration lies at the heart of transformation. We believe in the power of radical collaboration—bringing diverse voices together, fostering trust, and building equity and shared purpose. It’s about breaking down silos to create a future where everyone has a voice and where collective action drives truly inclusive outcomes.
One of our core commitments is to create and hold space where innovation and collaboration can flourish. We help organizations across health and social care, private and public sectors, municipalities, and public health tackle complex challenges by facilitating new ways of thinking and working together. The results? Solutions that are not only innovative but transformative.
My Career
Through 4C Impact, I currently hold several roles that allow me to advance integrated care, population health, and collaborative leadership. As Vice Chair of the International Foundation for Integrated Care (IFIC), I work alongside global leaders to drive the largest movement for integrated care practices worldwide. I am also a Co-Founder of IFIC Canada and the North American Centre for Integrated Care, based at the University of Toronto, which fosters innovative and collaborative solutions across North America. Additionally, I serve as Knowledge Broker and Mobilization Lead for the CIHR-funded Network for Integrated Care Excellence (NICE), amplifying initiatives such as the Transforming Health through Integrated Care (THINC) project. As Strategic Advisor to McMaster University’s Health Leadership Academy and Co-Director of the National Health Fellows and Collaborative Health Governance Programs, I focus on building leadership capabilities to match today’s complexities and opportunities. Previously, I held Chief Strategy Officer roles across health and social care, where I led large-scale transformations and built systems that prioritize integrated care, collaboration, and population health. These experiences have shaped my approach to system transformation and my commitment to creating lasting impact.
At this stage in my journey, I’m deeply committed to the leading edge of transformational change—how it happens, how it’s sustained, and how it shapes healthier futures. For me, this work is personal. It’s about creating conditions where relationships thrive, ideas spark, and solutions emerge organically. It’s about embedding equity, respect, and belonging into every step of the process.
So, whether you’re here to explore integrated care, seek guidance on collaborative leadership, or find a partner to navigate systemic challenges, my team and I are here to help. Together, we can create profound, meaningful change—not just for today, but for the future of health and well-being.
Thank you for visiting 4CImpact.org! Let’s connect and explore how we can transform challenges into opportunities, together.
Warm regards,
